Before You Begin
You will need:

Important Note: When evaluations are administered to students in class, it is absolutely essential that all students fill out the name of the instructor and the class on the evaluation form. There are two different types of evaluation sheets that departments are now using. One is a thin, green strip, called the ScanTron F-158, which does not have space for questions on the form, and the other is a full 8 1/2" x 11" sheet, with space on the form for departments to print their evaluation questions. This form is called the F-599. No matter what type of form your department uses for its questionnaires, you will need one F-158 form (the green strip) for each instructor you wish to evaluate. Collect all of the completed evaluation forms from each instructor. Make certain that all ScanTron forms are oriented properly, and that all student responses are marked in pencil. The ScanTron machine will not read forms that are not all going the same direction, nor will it register any response marked with a pen. Using a pencil, you can draw over the ink responses to make them visible to the ScanTron machine.

ISN FormsISN forms are what the computer uses to distinguish when the evaluation forms for one instructor stop, and when the forms for another instructor begin. To prepare an ISN form, take a ScanTron F-158 form, and fill in the following bubbles:

  1. At the top of the form, there is a 10-digit long section. In the section for the first digit, fill in both the number 2 bubble, and the number 3 bubble. Be sure you use a pencil for this and all other ScanTron marks.

  2. Leave the space for the second digit blank. In the space for the third digit, fill in the first digit of the instructor's ISN number. Mark this bubble ‘0’ if the instructor's serial number is less than 10.

  3. In the space for the fourth digit, fill in the second digit of the instructor's ISN number.


  4. Repeat the process for each instructor to be evaluated.

  5. After all ISN forms are made, I recommend marking the top of the form with a Hi-Liter or dark pen, so they can be easily distinguished from a top view.

Note: Each class must have an ISN number. If an instructor teaches more than one class, that instructor must be issued separate ISN numbers for each class. The ISN numbers must be continuous; they must run from 1 to the number of classes being evaluated, with no breaks in the sequence.

Master List
After you have made an ISN form for each instructor you are going to evaluate, you will need to make a master list of all instructors, by ISN number (print or download Master List example) This list should be submitted electronically (using a Word, WordPerfect Table or Excel file) by emailing it to evaluation@ucdavis.edu as an attachment.

The master list must include the following information for EACH class:

ISN – the number you just assigned to each class
Quarter – you can include multiple quarters
Year –you can include evaluations from multiple years
Instructor – the instructor’s name as you wish it to appear on the evaluation form
Role – Faculty, Instructor, Teaching Assistant, Lecturer, Professor, etc.
Class – INCLUDE the three-letter department identification as well as the class number
Section – if appropriate
Form Dept – this is associated with the set of questions used (see below)
Form ID– ‘a’, ‘b’, or ‘c’ (see below)
Enrollment – this allows calculation of the percent of class responding

Each department creates its own question form(s). The TRC can help with designing the forms and choosing and writing questions. Forms are identified by the three-letter department code and a form ID letter (‘a’, ‘b’, or ‘c’). The identification of the form is very important as it allows you to consistently communicate to me which sets of questions were used in which courses. If your department uses only one form, it would be identified as XXX A (for example, MAT A.) If your department uses different questionnaires in different classes, and you are unsure which form is ‘a’, ‘b’ or ‘c’, please phone us at 752-6050. Final Preparation
Now that you have assigned ISN numbers to all instructors, and made up your master list, you will need to prepare the entire set of evaluation and ISN forms for processing by the ScanTron reader. To do this, follow these simple steps:

  1. Place the ISN form for each instructor on top of that instructor's evaluation forms. It does not matter that the ISN forms that you created using the ScanTron F-158 form may not be the same size as the instructor's evaluation forms - just make sure that the timing marks (those little black marks along the left edge of the ScanTron form) of the evaluation forms and the ISN forms line up.
  2. Stack all the evaluation forms by ISN numbers. That is, the top of the stack of forms would be the ISN form #01, followed by the evaluation forms for instructor #1. After that would be ISN form #02, followed by the evaluation forms for instructor #2, and so on until the end of the evaluations.

    Note: Do not individually wrap each instructor's evaluations with rubber bands, paper clips, or other devices. The forms should be in one continuous stack.

  3. Place a printed copy of the Master List on top of the stack.
  4. Email a copy of the Master List to evaluation@ucdavis.edu
